For Members Who Are Planning Retirement
This page provides forms and information to apply for service retirement. This information is designed for members who are within 90 days of retirement.
Please read this important notice that concerns a possible amendment to SCERS’ Final Compensation Review Policy that could impact the calculation of your retirement allowance.
Application for Service Retirement
These are the forms you will need to apply for service retirement.
Member’s Affidavit
This form is mainly used to change your address, beneficiary, or marital status.
Tax Withholding
This form is used to change your Federal and California State income tax withholding amounts.
Authorization for Direct Deposit
This form is to sign up or change the direct deposit of your retirement warrant. It is also used when submitting the Service Retirement application.
Note: Banking information changes must be received by the 10th of the month* to process timely pension payments. Any request received on or after the 10th of the month will be processed the following month. Untimely notification or incorrect banking information will result in a payment delay of up to 20 days. Any rejected direct deposit will be issued by check and mailed directly to the address on file.
*Processing deadlines for the months of February and November occur earlier in the month.
Disposition of Accrued Sick Leave
This form is for management employees only to declare their intention to dispose of accrued sick leave. It is also used when submitting the Service Retirement application.
SCREA Membership
This form is to sign up for voluntary membership in the Sacramento County Retired Employees Association (SCREA) and authorize SCERS to take monthly deductions for SCREA dues from your retirement warrant.
Disability Retirement Application
Use the Disability Retirement Application and Instructions to submit your Disability Retirement Application.
For more information, refer to the Disability Retirement Procedures of the Sacramento County Employees’ Retirement System which govern the submission, review, and evaluation of Applications; the adjudication of Contested Applications; and the Board’s decisions on Applications and judicial review thereof, in accordance with the County Employees’ Retirement Law of 1937 (Government Code Sections 31450 et seq.), applicable case law, and SCERS Bylaws.
Dissolution of Marriage Guidelines
This document provides information and answers questions about the division of retirement benefits upon divorce or legal separation of an active member. This information is for members who are not retired or have not yet received benefit payments from SCERS.
